I had the same problem with my elbows on side planks. My solution is to fold a towel into a thick pad for my elbow and forearm. I try to position my arm so that the weight is distributed along my forearm - not just on the elbow. Maybe an exercise cushion would help distribute the weight?

I do use a cushion.  It makes side plank on my knees bearable, but not the regular.  I should try some different arm positions, I guess.  I haven't yet found one that works.  I think it might just be a weight-per-square-inch problem.
