Justa Verdi
Hi! My grandfather has been sick for months now. He is too weak to travel and his clinic is quite far from his place. I heard about this chronic care management. So, how does this work? Anyone who knows? Please help! Thanks in advance! Quote Link to comment
Igaduma
I suppose that depends on where you live. Quote My Profile | I must not fear. Fear is the mind killer. Holly26
Chronic care management is basically health care providers helping patients with chronic diseases to understand their condition and live successfully with it. It also depends on the condition of the patient. Quote Link to comment
Justa Verdi
I am referring to this software. I have read the link and it says that you can only be considered for chronic care, if you have at least 2 or more chronic conditions. Anyway, I will call the provider to verify.
