Please Critique!

I've been having trouble with my deadlift as I increase weight. I think this is do to my form. From the video it looks like the weight pulls me forward and this puts stress on my lower back, but I have no idea if (a) that's true and (B) what to do about it if it is!

Can those of you who know how to deadlift take a look at my form and let me know what you think and how to fix what I'm doing wrong?

Looks like when you set up your shoulders are slightly behind the bar. They should be ahead of the bar (think of a 10 degree angle between the bar and the shoulders. See here for a picture of what I mean.

Tried to capture what I am thinking from your video. The camera is tilted so it plays tricks on the lines.

Why is it everyone starts with hip extension and then stands erect from a slightly bent over position? One reason I made decent gain in the deadlift is I'm all set up and hip extension. My head leads and my hips follow. One smooth motion.

I always end up referring back to this video because it's the perfect deadlift technique for someone of normal stature:
